You will need to contact the plan administrator for your 401(k) account. They are required to provide you a Form 1099-R if you made a withdrawal from the 401(k), Not a Loan, in 2023.

You need the 1099R. If the bank or financial institution did not send you a 1099R you need to look for it on their web site or call them to ask how to get a copy. If you knew how to take money out of the account, you must know how to contact them.

To enter your retirement income, Go to Federal> Wages and Income>Retirement Plans and Social Security>IRA 401 k) Pension Plan Withdrawals to enter your 1099R.
