Form 8606 instructions say do NOT report a 100% re-characterized "distribution" (transfer) from a Traditional IRA to a Roth on 8606, just attach an explanation. How do I?

W-2 form box 14

These are not employer held IRAs. There is no box 14 on our W-2.
