Form 1040 Schedule 4 Where does line 57, 59 and 62 come from? All I get is "this form is fully calculated". The question is: from what?

Form 1040 Schedule 4 Where does line 57, 59 and 62 come from? All I get is "this form is fully calculated". The question is: from what?

Line 57 is Self- employment tax.  It comes from Schedule SE, which is generated from your net income on Schedule C.

Line 59 is Additional tax on IRA's, other qualified retirement plans, and other tax-favored accounts.  It comes from Form 5329, if required.  If not, it's just a calculation.  You're being charged this tax because you took a distribution from a retirement plan.

Line 62 is taxes from Form 8959 Additional Medicare Tax - this is an income-triggered tax and Form 8960 Net Investment Income Tax - another income-triggered tax.  These two forms are related to the Affordable Care Act aka "Obamacare."
