Do I need to include Wisconsin WT-4 (Employee's WI Withholding Exemption Cert) form with my application for a hardship withdrawal from my 401K or something else?

Regarding State Income Tax the form says: If applicable, I must attach my State income withholding form to make tax elections when required. Does Wisconsin require a document and if so is it W-204 WT-4?
