What does that mean? Which choice is best on the drop down menu asking for kind of retirment income?

Code 4 indicates that you were the beneficiary of the retirement plan due to death  It is not taxable by PA (however a code of 4D is!).  Choose I'm eligible, plan eligible to insure that particular distribution is not taxable by PA.
