@bowtieman8 I think I may have misunderstood your original question... just to verify is she expecting a 1099-G for state unemployment or a 1099-Misc for self-employment?

She was employed in Illinois  the company closed , the company is based in Texas . she did receive unemployment , but we did not receive a 1099 form .

Some states do not mail out Form 1099-G and you must obtain one via online. 

For Illinois, you can look up your 1099-G here: http://www.ides.illinois.gov/Pages/1099G%20form.aspx
