1. Si vous payez également la prime de votre conjoint, ouvrez le formulaire INFOWS et répondez Oui à la question « Annexe K - Voulez-vous payer la prime payable en vertu du Régime d’assurance médicaments du Québec de votre conjoint? ».

   Pour trouver ce formulaire, utilisez la fonction Recherche formulaire et entrez le mot clé infows.
