Yes, if you withdrew from your 401(k) in 2017, you should receive a 1099-R tax statement due to be sent out no later than January 31,

To enter a 1099-R:

·         Type in 1099-r in the search box, top right of your screen, then click the magnifying glass

·         Click the jump to 1099-r link in the search results

·         Follow to prompts and online instructions

A plan withdrawal is not reported on a W-2 form.
