You should receive a Form 1099R for this distribution. You can contact the company that handles your 401(k) and ask them for a copy of this form; you might be able to access it electronically. You will need this form in order to file your tax return. Once you receive this form, report the information in Turbo Tax.

To enter the Form 1099R in to Turbo Tax:
- Log in to
your Turbo Tax account
- click on
"take me to my return"
- click on the
"federal taxes" tab
- click on
"wages and income"
- choose
"show more" and/or "see list of all income"
- scroll down
to "investment income"
- click on
"IRAs, 401(k), etc"
- enter the
information from your 1099R
- Turbo Tax
will ask you questions about the distribution
