1099-INT Box 17

When completing an 1099-INT form, TurboTax requires me to put a value for box 17 since box 15 (State) and box16 (State Identification no.) are populated. The 1099-INT, I received from my bank, has box 17 empty (box 15 and 16 have values).

Has anyone else faced this?

Should I just clear the values from boxes 15 & 16 in Turbotax  and submit that 1099 form?

Only input the name of the bank or fiduciary and the amount of interest.  Ignore the other boxes.
