I should have been more clear. My self-employment net income was negative. But I had W-2 wages in excess of $65,000. Based on more research that I have done, it looks like I can contribute $8000 ($7000 + $1000 catchup, I'm 63) to my Roth IRA. But I cannot contribute anything to my Individual Roth 401K either as an employee or as an employer. Does this sound correct?

When you enter a 1099-R in TurboTax that has an RMD requirement, you get to a screen Let's get more information about your Required Minimum Distribution (RMD).  If the withdrawal is equal to the RMD, then you enter the Box 1 amount in the RMD due by Dec. 31, 2025 box.  On the next screen How much of this distribution applied to your December 31, 2025 RMD? you would check The entire distribution applied to the December 31, 2025 RMD, then Continue.  This will give TurboTax the information it needs to show that your distribution will be free of a penalty.

I am a live-in IHSS provider for my family member.  My income is tax exempt and it's on box 12a-II. Box 1 is $0.  My W2 shows a "QUAL OT" amount on box 14.  Do I need to include it? I am inputting W2 manually.   I believe the OT deduction does not apply to me because my income is non-taxable.   I am filling jointly with my spouse who was a W2 from his employer.

And I do thank you for the very detailed and useful answer

Thank you, I was able to manually correct it, but actually it didn’t need correction. The import did put things into the correct field. So I went ahead and filed it now what I need to do is see if the mistake had happened on the 2024 return usually, at least for me, each year my numbers are about the same in various boxes of the return so I was a bit surprised that there was such a difference, and the values were essentially flipped. I just assumed this year was a mistake, but it isn’t. So now I need to figure out how to go back and look at 2024 and what I have to do if it was a mistake,

There is no such thing as a "traditional Roth". There is either a "traditional IRA" or a "Roth IRA".   So are you contributing to a traditional IRA and are wondering why there is no deduction allowed?   Please see "Retirement topics - IRA contribution limits" as a starting point.

For the Federal return, mail to them the Federal 1040-X and the 1099-R Copy B to the IRS.  For the New Jersey return, mail them the NJ amended return, and a copy of the Federal 1040-X along with their 1099-B Copy 1.   Be sure to sign and date the amended forms for both of those before sending them in.  TurboTax would have also given you mailing addresses for each of those returns also.
