When entering your Form 1099-R information, on the "Enter your 1099-R provider's info" screen, you must indicate "This 1099-R belongs to" and then click the down arrow to select the taxpayer. If that is not done, TurboTax may combine the 1099-R information.
First, since an incorrect penalty was triggered, it is highly recommended that you delete your current 1099-R entries and the incorrect penalty Form 5329 to refresh the data:
Go to Tax Tools on the right hand panel inside of TurboTax
Click on the down arrow and scroll down to Tools
Click on Tools and select Delete a form from the menu items that appear in the Tool Center
Click on the trash can icon to the right of the Form 1099-R you would like to delete, then delete Form 5329
Next, re-enter the Form 1099-Rs following the steps below:
Go to Income on the left panel within your TurboTax account
Click on the Review to the right of IRA, 401(k), Pension Plan Withdrawals (1099-R)
Click on the +Add a 1099-R
Continue through the questions until you reach, "Enter your 1099-R provider's info"
Select your name from the drop down menu on the "This 1099-R belongs to" box (first box on top)
Enter the information from the original Form 1099-R that you received
When entering your Form 1099-R information, if RMDs apply to your Form 1099-Rs, follow these steps to indicate you took your RMD:
Return to Income, then click the down arrow to the right of Retirement Plans and Social Security
Select IRA, 401(k), Pension Plan Withdrawals (1099-R) in the Federal Income section
Click on +Add a 1099-R and input the 1099-R from which you took your entire RMD
Click Continue as you complete the interview entries
"Let's get more information about your Required Minimum Distribution (RMD)", enter the amount of your RMD (no extras) that was due by December 31, 2025, Continue
On the next screen, "How much of this distribution applied to your December 31, 2025 RMD?", enter your RMD (no extras) that you entered in the prior screen, Continue
When you reach, "Review your 1099-R summary, Continue (not the pencil)
When you reach, "Let's go over any required minimum distributions (RMD)", Continue (not the pencil)
If one of your Form 1099-R distributions covers all RMDs for 2025, you can enter the full amount of the RMDs (no extras) in the first Form 1099-R distribution above and then skip the second Form 1099-R questions about RMD by entering zero (0) at step 4 above and just Continue through the remaining questions. If asked, "Did you miss your RMD?", answer "No".

Log out of your return and try one or more of the following:
Don't use Internet Explorer.
Clear cache and cookies,
Sign in using a different browser.
Sign in using a different device.
Log back into your return.
Enter the information again.
Desktop version:
Delete the form and worksheets
Save your return while closing the program.
Update the program
Open

This is really great information, and I appreciate your thoroughness! I certainly understand why the 1099-B would result in owed taxes. In my excitement to file my taxes early, I neglected to give the investment firm time to send this form out. The state refund I received is from Maine. When I look at the actual amended return, it the form shows that taxes are owed, but doesn't actually say they are due (which is strange), although there are calculations for tax already paid (I assume, via the W-2s) as well as a negative amount in line 26 where it says "If this is an amended return, enter overpayment, if any, on original return or as previously adjusted." Not sure what this means. I did actually file two amended returns because of a Schedule K-1 that I also received late.

You are right about the very particular rules; but, I do want to comment that the earnings do rollover federally tax free. Which is what makes it different from a withdrawal. As long as you follow the annual and lifetime rules, there is no federal tax or penalty on the earnings.
For the ROTH, it is a clean slate and the entire rollover becomes the basis.
Box 4b is a rollover to ROTH. It is either nontaxable in its entirety or there is a taxable component.
A nontaxable 1099-Q should not be entered. The program wants to tax it. Tuck the 1099-Q into your tax folder. IRS Publication 970, Tax Benefits for Education states that nontaxable distributions should not be entered.
Taxable 1099-Q should be entered. While entering, under Other Form 1099-Q Information, select that it was a rollover to ROTH IRA. Continue answering the questions.

Since you now have to log into an Intuit Account all your downloads and licenses are listed there (even if you bought it from somewhere else) so if you need to download to install it again or install on another computer, even on the other operating system (Windows, Mac). And the download will always be up to date with all the updates.
