Are you referring to health insurance in regards to filing a US tax return? If you are a resident of a foreign country you are exempt from having US health insurance reported on a federal tax return.

Who is asking for a fiscal identification number?

Your Tax Identification Number (TIN) is your Social Security number.

I got married in November 2023 to my wife who lived overseas. We are filing taxes jointly for 2023 and treating her as “Nonresident Spouse Treated as a Resident". However, California is charging us a penalty for her not having insurance
