Assuming you are referring to California Form 568, Limited Liability Company Return of Income, if this is for a single member LLC filing a Schedule C, TurboTax will create Form 568 based on the answers to the questions entered in the Schedule C section and the California interview.
You can preview your return before filing to review the forms that TurboTax has prepared from your entries and to find out how your taxes were calculated. See here for details.
@MonikaK1 I realized I needed this form after filing. I have gone through the pdf and I couldn’t find it. Would it be possible to generate the form afterwards and just mail it?
Yes, but California requires a waiver be included with Form 568 when a taxpayer prints and mails Form 568. Here is the link to the CA website that relates to waivers. Scroll down the page and select Start Waiver to get your waiver request.
Business Entity E-File Waiver Request
Ordinarily, in the CA section of TurboTax, LLC owners should see the following two screens relating to Form 568: California requires that you e-file Form 568 and Creating Form 568. If you used TurboTax online, to double check that this form was not prepared, you might consider going back into your return and reviewing the forms that were prepared in connection with your return. Here are the steps to follow to view your list of forms. The steps are the same ones to delete a form; however, you will not be deleting a form, just viewing the list.
Because you have already filed your return, the above steps may not work as they would in a pre-filing situation. If that happens, then when you sign back into your account, select the option to add a state. You won't actually be adding a state, but this is a "back-door way" to re-open your return in TurboTax. Adding a state is an option in the My Info section of TurboTax and can be found at the bottom of the screen where it says Other state income. Just select Edit, and pick any state in the drop-down list. Once your return is open, then follow the above steps.
If you used TurboTax CD/download, to review the forms prepared in connection with your return, when you sign back into your account, select Forms in the upper right of your screen. Your list of forms should appear in the left margin.
@GeorgeM777 I followed your steps and I can confirm that form 568 wasn’t created. Is there a way to go back in to generate it?
If you have already e-filed your California return, you may not be able to generate Form 568 after the fact. But the steps given above by GeorgeM777 are your best option. Add a state and review your answers to the California state interview questions.
I viewed the forms that were generated but I haven’t filed yet and it looks like form 568 was not generated. How can I fix this so I form 568 is included? Thank you.
You may have missed those questions in the State interview (assuming you have a Single-Member LLC and file Schedule C with your federal return).
Go to the State Tax tab. Follow the interview and look for two screens relating to Form 568: California requires that you e-file Form 568 and Creating Form 568. Continue through the remainder of the state section. Form 568 should now be shown in the list of California forms.
I have gone through the CA Interview several times but was never shown the "California requires that you e-file Form 568" nor the "Creating Form 568" screens.
I have a Single Member LLC, generated Schedule C with my Federal return. I am using the Premier version on desktop and have updated the software.
Please help! If I don't add the form, I don't know how else to pay the $800 annual fee and then I'll be penalized.
Thank you.
Here’s how to create California Form 568.
Here's what my California screen looks like. I don't see how to Edit the Business Summary.
Is something not correct in my Federal filing?
You should be able to prepare Form 568, Try the following workaround.
Then, the next several screens are there for you to fill in details for Form 568. The first screen should be the only one with missing information. Scroll down the page until you see fields that are highlighted which need your input (they are pink on Windows versions of the program). As you scroll through the page, take a look at line 7. It should be filled in with $800.
Click Continue to progress through the rest of these screens. Then check the Forms Mode version of Form 568 to see if it looks correct.
If this does not work for you, please respond back.
@HopeS That worked! Thank you very much.
A feature request would be not to have to use a work around and be automatically prompted to fill out the form, once the Federal knows that I have an SMLLC and a Schedule C is generated.
Once again, for the detailed instructions and a working solution!

Hi @HopeS -
I followed your very helpful instructions on how to fill in the inputs for Form 568. After I filled in the inputs, there was a button that said "Print Form 568," which I did. However, I was expecting to see the compelted Form 568 in my return so that it will be submitted with the rest of my return, but it is not included in my return preview that I see in the Print Center in Tax Tools. Is there a reason it is not being generated as part of my return? This is my first year filing with my LLC, though I do not have any LLC income for 2022. I am using Turbo Tax Premium.
Thank you!!
has the proces for this changed? i cannot find a place to list single member llc in assets page and in my list of forms in the tax tool section the form isnt there. it is super frustrating as my LLC was suspended last year for this missing form.
