What is schedule 4?

Schedule 4 is a new form this year that may be needed along with your Form 1040. It is used to report "other taxes" such as:

Self-Employment Tax
Household Employment Taxes
Additional Tax on IRAs or other Qualified Retirement Plans

What is schedule 4?

Could this be because I did not have medical insurance? Because all of the other said things do not apply.
