I filed form 9465 to request an installment agreement, but then applied online at IRS.gov and was approved. Do I need to file an amended tax return and remove Form 9465?

The form I filed with my tax return had a different installment plan request ($500 per month via mailed check) than the one I was approved for online ($800 per month direct debit from my checking account).

No need to amend. The F-9465 has no effect on your tax return info. It's only used when one owes taxes and cannot pay in full . Since you made the direct debit agreement (which is the IRS preferred method) you're in good shape. When someone finally gets that F-9465 to process, they'll see you already have an approved installment agreement. FYI.
