Yes, you will include a copy of your W-2 with Massachusetts Form 1 (Massachusetts income tax form)

If you are mailing your return, TurboTax will give you instructions and print everything you need to paper file your return.

The Massachusetts signature block is at the bottom of page 1, just after question 3.
