I received a form 44444 from my employer with changes to box 12B. It was previously reported with 2499.84 and now is $0. Do I need to amend? How will this affect me?

Yes, you should amend, becasue Code B in box 12 of your W-2 represents uncollected social security and Medicare tax on tips. This means your taxes could be increased by the incorrect figure placed on your original W-2. Look on your Form 1040, Line 58 to see how this incorrect figure affected your original return.
