Can't figure out how to print my IL state form that I need to mail in.

The only form I can find for IL it says "Do Not File Form Not Complete"
I believe I have to file but I only made $36 from a settlement, from a company I used to work for.

Also I am not a resident of IL, I am a resident of MN.
