You may obtain information about the amount of money you have remaining on your Card by calling (888) 285-4169 or the number on the back of your Card. This information, along with a 12-month history of account transactions, is also available at www.turboprepaidcard.com. You also have the right to obtain at least 24 months of written history of account transactions at no charge by calling (888) 285-4169 or the number on the back of your Card or by writing us at:
Written History Request, P.O. Box 5100, Pasadena, California 91117.
Please include your name and Card number. You will not automatically receive periodic or paper
statements for your Card.

Ignore the prompt asking for your card number. Choose #1 for lost, stolen, or cards never received, then choose #2, enter SS# if prompted and continue following the instructions.
