To clarify, you say your income is not near the requirement for the additional tax so, did your employer withhold TOO much Medicare tax? That would also generate Form 8959.

Is the Additional Medicare Tax form being calculated correctly? When married and filed jointly it appears to be calculating based on $200k instead of the $250K (per IRS).
