You are mailing your tax returns? The one that says either 1040EZ, 1040A or 1040 U.S. Individual Income Tax Return is your federal tax return. Attach your W-2 and any 1099R's to the left side of the form. Sign it at the bottom where it says "Sign Here" and write the date. If it is a joint return your spouse needs to sign it, too.
The return for your state will have the state's name on top. When you print out the returns it will print with mailing instructions and the addresses to mail them to. Your state might require copies of the W-2's and 1099R's as well.
It is not required, but it is a good idea to use a mailing service that will track it, such as UPS or certified mail so you will know the IRS received the return.
