The TIN number can also be found on some Payers recording statements just under the column headings.

The Form 1099-G is issued by your state. You will have to check your state tax authority website for the department required based upon what you are needing to enter on the tax return.
If the 1099-G is for unemployment comp, check your state's unemployment website for it.
