We were affected by the FEMA disaster listed and our insurance claim verifies that. You can use the deposit date as the date received. @BPhelps

Does the distribution have to be identified as a disaster distribution on any paperwork from the retirement account?
No - the 1099-R is not going to reference any disaster information. @BPhelps
