When is it normal for boxes 3 & 4 to be blank on a W2?

This is normal if you participate in OPERS.

The State of Ohio has its own retirement system. If you work for a government agency that participates in OPERS you do not pay into Social Security. Therefore, boxes 3 and 4 will be blank.
