What is a form tc-40 and why the ***** does it need it

Form TC-40 is the individual income tax form for Utah. This form would be used if you reside in Utah or if you have income from Utah sources.

IF the review/error check is complaining a bout the UT Employer ID number....

go back to every W-2, 1099-R, and Unemployment 1099-G, and for the state line in that form, re-enter that state number....

........BUT, end the number with a lower-case wth (not upper case WTH)
