What are additional efile cost when I used premier turbotax service? or price for premier includes free efile?

There are no e-file charges when using the TurboTax online editions, federal or state.

The TurboTax account fee are for the use of the software to assist in completing a tax return.  The account fees are the same whether the tax return is e-filed or printed.

Premier online edition - $70

A State tax return is an additional charge of $40 when using the Deluxe online edition or higher.

Turbo tax Premier does still include 5 free e-file returns?  I was told the free e-file was only for simple returns

The TurboTax online editions have no charge to e-file a federal or state tax return.  

The TurboTax desktop editions include 5 free federal e-files included in the purchase price.
