W2 MA state boxes 15-17

I filed Exempt on my Massachusetts W4. The boxes 15-17 are empty due to this. 

My tax preparer is insisting that the exempt status is only for Federal. She will not file my taxes because she said there must be information for the state in boxes 15-17. I tried to research this and am getting conflicting information.  My employer and ADP are insisting there is nothing they can do.
