w2-Box 12 reflects $6,100 code W which is incorrect. PayFlex shows Employer Cont. $1,000 and Employee Cont $3,450. Also, a Limited FSA Cont was $2,700. Which is not shown at all.

Your W-2 should have in box 12 with a code of W only those contributions that your employer made to your HSA plus what you made to your HSA through payroll deduction (in this case $4,450).
You need to ask your employer for a corrected W-2, especially since I suspect that you have Self only HDHP coverage, so $6,100 will cause TurboTax think that you made excess contributions to your HSA.
Note. Unlike HSAs or Archer MSAs which must be reported on Form 1040 or Form 1040NR, there are no reporting requirements for FSAs on your income tax return. See IRS Publication 962.
