Is there any state income tax withheld in box 15? If not, leave all of the state and local information blank, boxes 13 through 18.

Turbo tax keeps sending my back to gambling winnings, wanting me to enter a state ID number from the W2G but its empty, what do I do to move on?

If box 15 of the W-2G does not have any state income taxes withheld then leave boxes 14, 15 and 16 blank (empty).
