Took maternity leave this year and received 8 weeks of partially paid leave via insurance. Form I received from the insurance company is not a W2, where do I enter?

Insurance paid 60% of my salary for 8 weeks in one lump sum, I had taxes taken out of the total. The paper I have to report out is not a W2 from the insurance company but has most of the pertinent information. Where does this information go in turbo tax? If I don't have all the W2 information, can I still enter it as a W2?

If you paid the premiums through payroll deductions, then the payments you received are not taxable and would not be included on your return.  That may be the reason you did not receive a tax document. 

 

If your employer paid the insurance premiums, you should have received a W-2.  If this applies, contact your employer. 

 

If your payments were made by a third-party (insurance company), you may need to contact them about a W-2, especially since you mention you had tax withheld.  Since the payments replaced wages, they may be taxable.  You could enter the amount you received as 'Other Reportable Income' and also enter the tax you paid under Deductions & Credits > Estimates and other Taxes Paid

 

The problem with entering the info as a W-2 is that the IRS will not find a matching W-2 and may question your reporting.
