Tax form 1099B only allows 2 entries. I've got e entries. How put in a third entry with Code E?

I have a 109-B from a financial institution.  It has Proceeds with Box A, Box B, and on the reverse side, Box E.

I went into TT Deluxe for paring a 1040, found and opened 1099-B.  This created a form 8949.

My document from the financial institution has THREE areas, Box A , Box B, and Box E.

I can only key in two of these, not the third one , Box E info.

What's wrong with my procedure.  I need simply an additional 1099-B to use for Box E information, with same name info as on the first two.  This would make a total of three check boxes to be used.

I pulled up a blank 1099-B form and do not see Boxes A, B, or E so you may need to comment with what each line is asking for it what I provide below does not assist you with your question.

 

Here is a blank 1099-B.  Please review your form to ensure it is the same.  

Blank 1099-B form

TurboTax has the input screens for each of the items listed on the blank form above.  To enter this information, you would go into the Federal section of the program.

  1. Select Income & Expenses
  2. Select Stocks, Mutual Funds, Bonds, Other (This would be listed under Investment income if you do not see it.)

You will the following screen if you have already entered the information from your 1099-B here.  

Add more sales

 

Notice I have highlighted the "Add more sales" should you have more sales to enter.

 

The following screen shows the input section of the 1099-B stock sale.  There is a section that allows you to indicate you have other boxes on your 1099-B to enter.  Check this box if you do have additional boxes of information to enter into the program.

should say, "I've got 3 entries."
