Hi... For 2024, my employer contributed $1000 towards my HSA and I put in an additional $3150, which made it the maximum allowable for the year (of $4150)
My question is: Is there a tax document that tracks the $3150? I don't see any mention of it in my 2024-1099 SA from my HSA custodian.

Yes, Form 5498-SA will show the contributions made to your Health Savings Account (HSA) during the year. However, that form is not sent to you until later in the year for you to keep in your records.
Since your employer contributed to your HSA, that amount should already be reported on your W-2 in box 12 with code W. If your contributions were made through payroll deductions, those will also be included on your W-2.
Form 1099-SA reports only distributions from the HSA, not contributions.
