State tax coupons?

I've printed out all 88 pages of my return twice - for Fed/AZ/OR.  Only the fed taxes printed out the payment coupon but not the AZ and OR state taxes.  Any idea why?  TIA

You can generally pay without a voucher. When mailing your payment just be sure to include your social security number and tax year on the payment.  

For AZ, you can go here to either print a voucher or pay online.
