Should Box 20 - Locality Name be entered as the Locality Code Number listed on my W2 [26] or should it be the actual Locality Name [Fayette]?

It might not matter when using TurboTax since I do not see Fayette as a locality TurboTax services. 

Can you clarify which state that is in?

This will not have any affect on your return as TurboTax does not handle local tax returns in PA.    So, you can enter either the code or Fayette.  You will need to file your local taxes through Southwest Regional Tax Bureau.
