All of your income goes on the same Form 1040. If you have self-employment income you need online Self-Employed, and using that software you can also enter your W-2(s). Or you can use any version of the CD/download to enter self-employment income and your W-2 income. The CD/download has all the forms and schedules in all versions.

Using the TurboTax online editions you would need the Self-Employed edition to report both W-2 income and self-employment income.
If you are a self-employed sole proprietor or independent contractor who has not incorporated then your business is a disregarded entity for tax purposes. You will be completing a Schedule C for the self-employment income and expenses. Schedule C is included with and part of your personal tax return Form 1040 and is not a separate tax return.
