No you can print and mail the state return.  In the Online version the fee is to prepare the state return whether you efile or Print and mail.  In Deluxe and above the state fee is currently $40.  

In the Desktop program you get one free state program download to prepare unlimited state returns (in the same state).  You can print and mail state for free or each state is 39.99 to efile from the Desktop program.

No, there is no requirement that you file a state tax return when using TurboTax.  However, since the federal tax data transfers to the state return, it is advisable.  The state program when using the TurboTax online editions is an additional charge which is currently $40 when using the Deluxe edition or higher.
