I was wondering what is a schedule A form? Also where would I find it if I filed a 1040A in 2015?

Schedule A is for claiming Itemized Deductions (medical expense, mortgage interest, taxes paid, property taxes, etc.).

If you filed a Form 1040A for 2015, there would not be a Schedule A included with your tax return.  Schedule A can only be included along with Form 1040.
