Roth IRA

I took a qualified distribution from Roth. Is there a way to avoid question regarding contributions? I have had the account for numerous years and am unable to determine amount. Since distribution is qualified can I just bypass these questions?

Yes skip contributions. ,You only need to enter the 1099-R and answer the questions about disaster, RMD and public safety officer, that set of questions that comes after entry.
