Resubmit 1040 form with identity theft form?

I completed and efiled my tax return.  It was rejected due to identity theft.  When I send the identity theft affidavit form in, do I need to send another 1040 form?

If your return was rejected and no accepted by the IRS, yes, you will need to paper file.
