If you pay for the supplemental insurance policy with after tax dollars (meaning not before tax through an employer), then you won't be taxed on this income. If you received a form, let me know and I can provide more information.

This wasn't a reimbursement because of health costs you paid for, was it? Did they issue you any forms?

I only have the EOB with the claim number and the approved benefit. It was from a policy that the beneficiary purchased on their own and it is not an employer provided policy. My research is indicating the same conclusion that it is not taxable. Thank you for your assistance.
