I purchase health insurance through a former employer. As an incentive to encourage preventive care if I have an annual physical, then they will send me a check for $500. Is this income or a rebate. They sent a letter stating it is a taxable incentive payment but did not process it as a payroll check since I do not work for them. No W-2 or any other tax form will be provided. If it is income, how do I report it? Thanks for any help.
