in forms mode search for 1099-b worksheet. enter description as non-business bad debt cost $5,000.

cost basis not reported to IRS. for sale date the date it became totally worthless. acq date - various.

as a capital loss a maximum of $3000 will be deductible in 2019 the balance will carryover to 2020
