Assuming you are referring to Wisconsin Form 804, Claim for Decedent's Income Tax Refund, you can only attach a pdf if you are using TurboTax Desktop.
The entries will need to be made in the state interview section of the program.
- Log back into TurboTax Desktop and select the state tab.
- Proceed through the screens until you reach the page titled A few things before we wrap up your state taxes.
- Under the section titled Other situations, select start to the right of Required PDF Attachments.
- On the page titled Other Documents To Attach, type in Claim for Decedent's Wisconsin Income Tax Refund in the box to the right of Document name (note you will not be able to include spaces).
- Under the box for Purpose of Attaching, type in Decedent refund claim.
- Select continue. You will then see a page titled Looks like you need to attach a PDF. Select Continue.
- On the next page, you'll be prompted to upload your required attachment(s). Select the Get PDF blue button to attach your Form 804.
- Once you select continue, you will see a note saying "we've attached the PDF(s) to your return. You're all set."
- When you electronically file, your Wisconsin Form 804 will be sent with your state tax return.
