There was a change of control in a (non public & foreign) company that I used to work for, which led to an option termination consideration agreement. They just sent me a check, no W2 (no withholding) or 1099. How and where do I report this income?
Thanks for your help...

I left a job and exercised my NSOs a few months later. It was a cashless exercise where shares were redeemed to cover the costs and tax withholding. When I got my w-2 neither the income or the withholding was reflected on it. Where do I report the withholding as taxes already paid? How do I know how much of the withholding was for state, federal, Medicare, etc?
The taxes withheld are properly reported on your W-2 form already.
