Yes, you have to report the information on your Form 1099-B. You will use the information provided by your employer to adjust the what is reported on the 1099-B after you enter it. When you enter the Form 1099-B, you will be asked if it is Company Stock - answer yes. When prompted to import your Form 1099-B indicate you will enter it another way.
