No, do not enter your earnings from your life insurance annuity unless your account administrator sends you a Form 1099-R - IRS

I have a life insurance annuity on myself. Do I need to enter the earnings on my tax return?

I would call the company to find out if you'll be getting  the form.  if not, you'll have no way to know that.  Though technically, they are required to be issued by 1/31/2020.
