I filed form 1041 for a trust, chose summary entries:1 entry on form 8949 says '... see attached statement". I e-filed it. Where/when do I send the supporting 1099-B?

If you have completed the e-filing your 1041 tax return, then mail your statements along with Form 8453-FE with Form 1099-B and a statement of explanation to:

Internal Revenue Service

Attn: Shipping and Receiving, 0254 

Receipt and Control Branch 

Austin, TX 73344-0254

If you need a blank Form 8453-FE you can download this pdf, enter your address information.  The address is the e-file station and should connect your document with your 1041 tax return.

I don't understand what you mean by the "address is the e-file station". Wouldn't the EIN of the trust on form 8453-FE connect it to the return?

Yes the EIN will connect the return.  The term e-file station is simply a description of the address where e-filed returns send their 8453 required documents.
