If your only income for 2023 was Social Security you do not have to file a tax return unless you have a 1095A for marketplace health insurance. You do not have to explain to the IRS that you were in the hospital or that you could not work.
There is no need to tell the IRS why you did not work in 2023.
If the Social Security benefits are the Only income to be reported on the tax return, then there is no reason or need to file a tax return.
Unless you also received a Form 1095-A for your health insurance through one of the State Marketplace Exchanges or from healthcare.gov then you must file a tax return.
