Form 8959 even though I'm under the threshold

I am well below the threshold but TT keeps adding the form.  I've tried deleting the form but it keeps getting added back.  I've tried clearing my cache.  I've tried all the things in the previous threads, but it keeps happening.  I should not have that form.  How can this be fixed?
